membership fee, and feature a wide variety of benefits. These benefits may
include X-rays and exams at no copayment, and copayment levels at 30-50% off
our Usual, Customary, and Reasonable (UCR) rates. These plans may have no
deductible, no annual maximum, and may not require pre-authorization for
treatment.
